Care Coordination

We connect with families worldwide to share Els for Autism expertise. Families receive individualized guidance regardless of location, and our experts connect them with personally vetted resources in their immediate area.

Care Coordination at Els for Autism helps individuals with autism, clients, and their families navigate programs, services, and resources within the organization and through trusted external partners. Families reach out from across the country and around the world, and Care Coordination is designed to make accessing support clear, welcoming, and easy to understand.

Families and clients are connected with a dedicated point of contact who answers questions, helps identify needs, and guides them to the most appropriate programs or services. This service provides individualized guidance and trusted connections, helping families and clients feel informed, supported, and confident as they explore available options.

Care Coordination helps families and clients:
  • Learn about available programs and services
  • Identify the best fit based on individual needs and goals
  • Connect with appropriate team members or community resources
  • Understand next steps for enrollment or participation
How the Care Coordination Process Works

Families and clients can reach out through the website or by direct contact to inquire about services. During the initial conversation, a few brief questions are asked to better understand the individual’s needs and interests.

Based on this information, families and clients are guided to appropriate programs, services, or team members within Els for Autism, as well as to external resources when helpful. When needed, Care Coordination also includes support with next steps such as completing registration portals, submitting required documentation, or connecting with a program manager.

Information Collected During Initial Inquiry

To support appropriate service connections, the following information may be collected during the initial inquiry and securely tracked:

  • Full name
  • Age
  • Gender identification
  • Program or services of interest
  • Zip code or geographic area (state, country)
  • Race or ethnicity (optional, for demographic tracking)
  • Phone number
  • Email address
  • Current involvement in other programs or services, within Els for Autism or externally

Brief Intake Survey

A short intake survey may be completed during the initial Care Coordination conversation to help better understand needs, interests, and goals. This information supports personalized guidance and appropriate service recommendations.

Please note: Completing organization surveys helps secure grant funding that allows Els for Autism to continue providing essential Care Coordination services. Participation and feedback are greatly appreciated.

Resource Guide

As part of Care Coordination, families and clients may be referred to the Els for Autism Resource Guide, which includes internal programs and trusted external community resources to support informed decision‑making.

Care Coordination Team
